Below are the basic steps to file for shared parenting in Ohio: One parent must file a Complaint requesting the court enter a shared parenting decree. Both parents must meet and agree on the terms of a shared parenting plan, preferably with the support of a qualified family law attorney.
These include: A W-2 form from each employer. Other earning and interest statements (1099 and 1099-INT forms) Receipts for charitable donations; mortgage interest; state and local taxes; medical and business costs; and other tax-deductible expenses if you are itemizing your return.
Copies of your income statements (W-2s, W-2Gs and/or 1099s) if you have an amount on line 14 of your IT 1040 and/or an amount on line 11 of your SD 100. Copies of the required certificates and/or Ohio IT K-1s if you are claiming any refundable and/or nonrefundable business credits on your Ohio Schedule of Credits.
Fill out FORM 534A Name of submitter. Return address. Email and phone number of the submitter. Select an expedited option, if needed. Fictitious Name. Enter the DBA name to be registered. Name of registrant. Registrant entity's number (if the DBA would be under a business)
While you could file for divorce by not going to court in person, you'll need to go to court and attend a final hearing at the end of the process. This applies whether you are the spouse filing for the divorce or not. During the hearing, you'll be asked basic questions, and it usually won't take longer than 15 minutes.
Form 8 – Counterclaim for Divorce Without Children. (this is not required, unless the Defendant chooses to file a Counterclaim) Form 5 – Motion and Affidavit or Counter Affidavit for Temporary Orders. (not required, but should be filed if party wishes to have orders in place during the pendency of the case)
Default Judgment In this case, the divorce would proceed without the participation of the other spouse, and the court would make decisions regarding child custody, property division, child support, and alimony based on the information you provided.

A separate Ohio ST AR must be filed when requesting a refund for sales tax and a refund for consumer use tax. For all consumer filed refund claims, the following supporting documentation must be supplied: Copies of original invoices or similar documents indicating the amount of sales tax charged.
