Statement of Military Service

A Statement of Military Service is a document outlining the service history of an individual in the armed forces. It typically contains information such as a service member's enlistment date, rank, military assignments, awards, and decorations. The statement is used by veterans, service members, and their families for a variety of purposes, including applying for benefits, obtaining medical records, and verifying military service. There are two main types of Statements of Military Service: the DD Form 214 and the GB Form 22. The DD Form 214, also known as the Certificate of Release or Discharge from Active Duty, is issued by the Department of Defense (DoD) when a service member is discharged or retired from active duty. It contains information such as the service member's name, Social Security number, date of birth, branch of service, dates of service, awards, and separation or retirement information. The GB Form 22, also known as the National Guard Report of Separation and Record of Service, is issued by the National Guard Bureau for members of the National Guard and Reserve. It contains the same information as the DD Form 214, but also includes additional information such as drills, training, and mobilization information.
