Wisconsin Health Service Data Base Certification Application is a form that is used to request access to the Wisconsin Health Service Data Base (WHS). This form is used by healthcare professionals, researchers, and other individuals who need access to the WHS, which contains a variety of health and medical related information. The application must be completed in full and submitted to the Wisconsin Department of Health Services for approval. There are two types of Wisconsin Health Service Data Base Certification Applications: one for individuals and one for organizations. Individuals must provide personal information such as name, address, contact information, and proof of completion of a HIPAA training course. Organizations must provide a list of individuals requesting access, along with contact information and proof of completion of HIPAA training for each individual. Once the application is approved, individuals and organizations will be granted access to the WHS.
