Wisconsin Form ADV-H is a form that is used by investment advisers and representatives who are registered with the Wisconsin Department of Financial Institutions (DFI). It is a part of the Uniform Application for Investment Adviser Registration, which is used to register with the DFI as an investment adviser or representative. There are two types of Wisconsin Form ADV-H: Form ADV-H1 for investment advisers and Form ADV-H2 for representatives. Form ADV-H1 must include information about the investment adviser’s business and financial background, the names and addresses of the firm’s principals, the firm’s business practices, its investment strategies and portfolio composition, and any disciplinary information. Form ADV-H2 must include information about the representative's business and financial background, the names and addresses of the firm’s principals, the representative's business practices, the representative's investment strategies and portfolio composition, and any disciplinary information.
