Washington Air Conditioner/Heat Pump Pre-instruction Checklist and Start Up Affidavit is a form used by contractors and technicians to document the pre-instruction and start-up of a Washington state air conditioning or heat pump system. This form is required in order to demonstrate compliance with the Washington Department of Labor and Industries’ Residential Air Conditioner and Heat Pump Installation and Start-up Rules. The checklist includes a comprehensive list of items to be inspected and verified prior to system start-up, as well as a section for documenting any corrective action taken.
