Washington Sealed Confidential Guardianship Document Cover Sheet (CURSE) is a form used in the state of Washington to protect the confidential information of guardianship documents from public view. The CURSE form is used to seal guardianship documents in the court file, and the information contained within the documents is not disclosed to anyone other than the court and the parties involved in the guardianship proceedings. The Washington Sealed Confidential Guardianship Document Cover Sheet (CURSE) has two types: a standard cover sheet and an extended cover sheet. The standard cover sheet is used for documents that are not considered confidential, while the extended cover sheet is used for documents that contain confidential information, such as financial information, medical information, or social security numbers. Both cover sheets can be filed electronically with the Washington State Court System.
