A Washington Sealed Confidential Report (Cover Sheet) is a document used to protect the privacy and confidentiality of certain information. It is used to certify that the contents of a report have been kept confidential and have not been disclosed to unauthorized persons. The cover sheet is typically used to accompany sensitive reports prepared by a state government agency in the state of Washington. There are two types of Washington Sealed Confidential Report (Cover Sheet): the Standard Form 803 and the Standard Form 804. The Standard Form 803 is used for reports that are to be held confidential for limited periods, while the Standard Form 804 is used for reports that are intended to remain confidential indefinitely. Both forms require the report to be signed by the agency head and two designated agency staff members.
