The Washington Case Information Cover Sheet (CICS) is a form used by the Washington State Superior Court to provide information about a case. It is used to provide summary information about a case, including the parties involved, the case number, the location of the court and the judge assigned to the case. The CICS form must be completed and filed with the court before any other documents can be filed in the case. There are two types of Washington Case Information Cover Sheet: the "Standard" form and the "Supplemental" form. The Standard CICS form provides basic information about the case, including the names of the parties, the case number, the court location and the assigned judge. The Supplemental CICS form provides additional information about the case, such as the date of filing, the case type, and the attorneys involved.
