Vermont Release of Information (PC) is a legal document that must be completed and signed by an individual in order to allow a third party to access certain confidential information. This document is used to provide authorization to a specific entity to access private records, including medical records, financial statements, criminal records, etc. The Vermont Release of Information (PC) has two types: the Vermonter Release of Information Authorization and the Vermonter Release of Information Authorization with Signature Notarization. The Vermonter Release of Information Authorization is used to grant the recipient access to confidential information and is signed by the individual. The Vermonter Release of Information Authorization with Signature Notarization is used to grant the recipient access to confidential information, and is signed by the individual and notarized.
