The Virginia Cover Sheet For Filing Civil Actions is a form issued by the Supreme Court of Virginia that must be included with the Complaint or other initial pleading when filing a civil action in the Virginia court system. The Cover Sheet includes the names of the parties, the type of action, the cause of action, the relief sought, and whether the action is assigned to a particular court. There are two types of Cover Sheet: one for Circuit Court actions and one for District Court actions. The Cover Sheet must be signed by the attorney of record or by an individual if the individual is not represented by an attorney. This form must be submitted to the court clerk at the time of filing.
