The Utah Doing Business As (DBA) Registration Information Change Form is a document used by business owners in the state of Utah to make changes to existing DBA registrations. This form allows business owners to update their information with the Utah Division of Corporations and Commercial Code (UCC) such as address, name, and contact information. There are two types of Utah Doing Business As (DBA) Registration Information Change Form: the Name Change Form and the Address Change Form. The Name Change Form is used to update the registered business name with the UCC whereas the Address Change Form is used to update the business address with the UCC. Both forms must be completed in order to update the DBA registration information.
