An Appearance Form is a legal document used in court proceedings to inform the court and the opposing party of a person's intention to appear in court. It is typically used in criminal cases, but can also be used for civil matters. The form is signed by the defendant, and in some jurisdictions, also by the defendant's attorney. There are two types of Appearance Form. The first is an Appearance by Counsel Form, which is used when the defendant is represented by an attorney. The second is an Appearance in Person Form, which is used when the defendant is appearing in court without an attorney. In both cases, the form requires the defendant to provide their name, address, and contact information. It also requires the defendant to agree to appear in court on the specified date.
