This form is used for cases involving the removal of a party from the United States. It requires the petitioner to provide their name and address, the name and address of the party being removed, the reason for the removal, and the names of any witnesses or evidence to be presented at the hearing. The petitioner must also indicate whether they are represented by counsel and provide the name and address of their attorney, if applicable. The form has two versions: one for individuals and one for corporations.
