Tennessee Application for Certificate of Withdrawal Following Administrative Revocation is a form used by individuals in Tennessee who have had their driver’s license revoked due to an administrative action. This application is used to request a Certificate of Withdrawal from the Tennessee Department of Safety and Homeland Security. There are two types of Tennessee Application for Certificate of Withdrawal Following Administrative Revocation: Voluntary Withdrawal and Mandatory Withdrawal. Voluntary Withdrawal is for individuals who voluntarily surrender their driver’s license in lieu of further administrative proceedings. Mandatory Withdrawal is for individuals who have had their driver’s license revoked due to a court order or administrative action. The application must be completed and mailed to the Tennessee Department of Safety and Homeland Security with all required supporting documents. Upon receipt of the application and all supporting documents, the Tennessee Department of Safety and Homeland Security will review the request and issue the Certificate of Withdrawal.
