The Tennessee Notice of Intent to Redact (TN-NIR) is a legal form used by the Tennessee Department of Health to protect the privacy of healthcare providers and other entities when releasing medical records. It is used to notify the patient (or their legal representative) that certain health information may be redacted from the records before they are released to the requesting party. The TN-NIR outlines what information is to be redacted, the reason for the redaction, and the legal authority supporting the redaction. There are two types of TN-NIR forms: the TN-NIR-A for medical records and the TN-NIR-B for mental health records. Both forms require the patient's signature before the redaction can be completed.
