The Rhode Island Appeal Form-Division of Motor Vehicles Operator Control is a form used to appeal a suspension or revocation of an operator’s license or privilege to operate a motor vehicle in Rhode Island. It is used for a wide variety of reasons, such as failure to pay fines, traffic violations, or license revocation for medical reasons. There are two types of Rhode Island Appeal Form-Division of Motor Vehicles Operator Control: an Appeal for Suspension or Revocation of Operating Privilege and an Appeal for Refusal of Renewal or Reinstatement of Operating Privilege. Both forms are available online from the Rhode Island Division of Motor Vehicles or from the DMV office in your area.
