Pennsylvania Notice of Appeal From District Justice Judgment is a legal document that is used to initiate an appeal of a judgment from a district justice court in Pennsylvania. The document must be completed, signed, and filed with the court clerk of the district justice court within thirty (30) days of the district justice’s decision. The Notice of Appeal must include the name, address, and phone number of the appellant; the name, address, and phone number of the appealed; the date of the district justice court’s decision; the court’s docket number; and a statement of the reasons for the appeal.
