Exemplification of Probate Record Request Form The Pennsylvania Exemplification of Probate Record Request Form is used to request and obtain a certified copy of a death certificate or probate records from the Pennsylvania Department of Health Vital Records Office. The form can be used to request records held by the Department or records that have been filed with the Register of Wills in the county in which the decedent resided. There are two types of Pennsylvania Exemplification of Probate Record Request Form: One is a "Certificate Request Form" for requesting copies of death certificates; and the other is a "Probate Record Request Form" for requesting copies of probate records. Both forms include information regarding the decedent, the applicant, and the requested record, as well as a fee for processing the request.
