Pennsylvania Consent of Adopted is a legal document that provides consent from an adopted for the release of identifying information to another party. This document is used to facilitate the exchange of non-identifying information between the adopted and the adopted’s birth parents or other family members. It can also be used to obtain the adopted’s original birth certificate. Two types of Pennsylvania Consent of Adopted are available: Standard Consent and Limited Release Consent. Standard Consent allows the adopted to allow their identifying information, including their name, date of birth, place of birth, and other identifying information, to be released to a specific person or entity. Limited Release Consent allows the adopted to allow only their name and birthdate to be released. Both types of consent must be signed by the adopted and notarized before they can be submitted.
