The Oregon Affidavit In Support of Registration Of An Out-of-State Child Custody Determination (5C) is a form used to register an out-of-state child custody determination in Oregon. This form is submitted to the court in the county in which the child resides. It must be signed by both parents and must include a copy of the out-of-state child custody order. The affidavit must also include a statement of facts regarding the child's current situation such as the child's residence, school attendance, medical insurance, and any other relevant information. The affidavit must be notarized and accompanied by a filing fee. There are three types of Oregon Affidavit In Support of Registration Of An Out-of-State Child Custody Determination (5C): Original Affidavit, Modification Affidavit, and Termination Affidavit. An Original Affidavit is used to register an out-of-state child custody determination for the first time. A Modification Affidavit is used to modify an existing out-of-state child custody determination. Finally, a Termination Affidavit is used to terminate an existing out-of-state child custody determination.
