It is most commonly used in civil court cases in the state of Oregon. In Oregon, the Affidavit of Service is typically filed with the court by the person or entity that provided service of process, such as a professional process server or private investigator. The Affidavit of Service must include information such as the date, time, and address of service, as well as the name and address of the person or entity served. There are two types of Oregon Affidavit of Service: (1) Personal Service, where the document or notice is delivered directly to a person or entity; and (2) Substituted Service, where the document or notice is delivered to an alternate person or entity, such as a family member or employee.
