The Oklahoma Accredited Investor Exemption Supplemental Information Form(.pdf) is a document used by a company to provide additional information to the Oklahoma Corporation Commission for the purpose of obtaining an exemption from Oklahoma securities registration requirements. The form is used by companies that have already qualified as an Accredited Investor under the Oklahoma Accredited Investor Exemption. It is used to supplement the information previously provided in the Accredited Investor Exemption application. There are two types of Oklahoma Accredited Investor Exemption Supplemental Information Forms: one for corporations and one for partnerships. The form includes information such as the company's total assets, liabilities, and net worth; the company's business and purpose; the company's officers and directors; the company's history; and whether the company is a foreign corporation. It also includes a statement affirming that the company is an accredited investor and has made all required disclosures.
