The Oklahoma CM/ECF Registration Form (also known as the Oklahoma Centralized Electronic Filing System Registration Form) is a document used by legal professionals to register for access to the Oklahoma Centralized Electronic Filing System (CM/ECF). The CM/ECF system is used to electronically file documents with the Bankruptcy, District, and Appellate Courts of the United States. The Oklahoma CM/ECF Registration Form is divided into two parts: the first section is for attorneys, and the second section is for non-attorneys. In the first section, attorneys must provide their name, bar number, address, telephone number, email address, and address of their law firm. In the second section, non-attorneys must provide their name, address, telephone number, email address, and the name of their court-appointed representative. Once the form is filled out, it must be submitted to the Oklahoma Centralized Electronic Filing System Administrator. Upon approval, the applicant will be granted access to the CM/ECF system.
