The New York Request For Judicial Intervention Addendum (RJ) is a document that is used when filing an action in a New York State Supreme Court. It is a required form that must be submitted along with the Complaint or other initiating pleading. The RJ provides the court with the necessary information to assign a judge, determine the filing fee, set a return date, and enter a judgment or order as the case progresses. There are two types of New York Request For Judicial Intervention Addendum: the RIGA for actions and the RISC for CPR Article 78 proceedings. The RIGA is used when filing a Complaint or other initiating pleading and the RISC is used when filing an Article 78 petition. Both forms must be completed in full, signed, and filed with the court. The RJ must be filed prior to any responsive pleading being filed.
