New York Proofs of Identity for Registration and Title are documents that must be provided to prove one's identity before a motor vehicle can be registered and titled in the state of New York. The types of documents accepted as New York Proofs of Identity for Registration and Title include a valid, unexpired driver license or non-driver ID issued by the New York State Department of Motor Vehicles (DMV); a valid, unexpired photo ID card issued by a state or federal government agency; a valid, unexpired U.S. passport or passport card; or a valid, unexpired immigration document issued by the U.S. government. All documents must show the person's full name, date of birth, and address. The DMV may also accept other documents as valid proofs of identity, such as valid foreign passports, military IDs, or other identity documents issued by the U.S. government or foreign government.

You must meet the following criteria to apply for an IDNYC card: 1. At least 4 points of documents with at least 3 points proving identity and at least 1 point proving residency in NYC. 2. At least 1 of the documents submitted must have a photo, unless applicant is accompanied by a caretaker.

Use the form Vehicle Registration/Title Application (PDF) (MV-82), available at any motor vehicle office, by request from a DMV Call Center or by download from the DMV internet site. You must pay the registration and license plate fees, any appropriate sales tax, and the fee for a title certificate $50.00.

If you are applying for a REAL ID or Enhanced document you must provide two proofs of New York State residency. A New York State license, permit or non-driver id card, a recent bank statement, or a recent pay stub showing your current New York State address are just some of the acceptable proofs of residency.
