New York Application For Admission To Practice As Attorney And Counselor-At-Law (Forms) is a series of forms required to be filed with the Appellate Division of the Supreme Court in the state of New York in order to practice law as an attorney and counselor-at-law. This includes the forms for admission to the bar, the oath of office, and the affidavit of admission to practice. There are separate forms for admission to the bar for applicants who are New York State residents, non-residents, and foreign lawyers. The forms for the oath of office and affidavit of admission must be completed and submitted to the Appellate Division along with the appropriate fee. All forms must be completed in full and signed in the presence of a witness. Once all forms have been submitted, the Appellate Division will decide if the applicant has met the qualifications to practice law in New York.
