The New York Affidavit of Service [Personal Delivery] (CIV-RC-94) is a legal form used to document the personal service of a summons, complaint, notice, or other court document. It is used to certify to the court that the document was personally served to the intended recipient. The Affidavit of Service must be completed by the process server and notarized before it can be submitted to the court. There are two types of New York Affidavit of Service [Personal Delivery] (CIV-RC-94), an individual service and a business service. The individual service form is used when the recipient is an individual, while the business service form is used when the recipient is a business or other legal entity. Both forms require the process server to provide the following information: name and address of the recipient, date and time of service, manner of service, and a detailed description of the document served.
