This form is used when an attorney wishes to terminate their ability to e-File in the New York State Courts. This form must be filed with the court in which the attorney is authorized to practice. The form must include the attorney's name, address, telephone number, and email address. This form also requires the attorney to list the cases in which they wish to revoke their e-Filing authorization.
