New Jersey EAP — Enrollment Form is a document used to enroll in the New Jersey Employee Assistance Program (EAP). The EAP is a free, confidential program that provides counseling services and resources to eligible state employees and their families. The form requires personal and contact information such as name, address, date of birth, and phone number, as well as information related to the employee’s employment status and health insurance coverage. It also requires signature of the employee and the employer to accept the enrollment terms and conditions. There are two types of New Jersey EAP — Enrollment Form: one for active employees and one for retirees.
