The New Jersey Report and Award Form of Arbitrator(s) — Auto / Personal Injury is a legal document used to report the outcome of an arbitration hearing regarding a dispute between two parties over an auto or personal injury claim. The form is used by arbitrators to award damages to the claimant and to provide a summary of the facts and issues of the case. It is typically used in cases where the parties have agreed to forgo a trial and instead, use arbitration as a means of resolving their dispute. There are two types of New Jersey Report and Award Forms of Arbitrator(s) — Auto / Personal Injury: 1) Death, Property Damage, and Bodily Injury Claims; and, 2) Wrongful Death Claims. The form must be completed and submitted to the court for approval.
