New Hampshire Form D-2 Articles of Charter Surrender is a form used to dissolve a business entity in the state of New Hampshire. The form must be completed and filed with the New Hampshire Secretary of State in order to legally terminate the entity. There are two types of New Hampshire Form D-2 Articles of Charter Surrender: Articles of Charter Surrender for Corporations and Articles of Charter Surrender for Limited Liability Companies. The form must include the name of the entity, the date the Articles of Charter Surrender were executed, the signature of the entity's statutory agent or registered agent and the signature of the entity's president or other designated officer. The form must also include a statement that the entity is in good standing and has paid all taxes, fees and assessments due. Once the form is completed and submitted to the Secretary of State, the entity will be officially dissolved and its charter cancelled.
