There are two types of name change applicants in Minnesota: Adult Name Change Applicants and Minor Name Change Applicants. Adult Name Change Applicants must be over 18 and must complete a Petition for Name Change Form, a Name Change Affidavit, and a Name Publication Form. All forms must be completed, signed, and notarized. The applicant then files these documents with their county court. Minor Name Change Applicants must be under 18 and must complete a Petition for Name Change Form, a Name Change Affidavit, a Name Publication Form, and a Minor's Consent Form. All forms must be completed, signed, and notarized. A parent or legal guardian is also required to sign the Minor's Consent Form. The applicant then files these documents with their county court.
