Michigan Request to Review Accommodation is a form used to ask the Michigan Department of Civil Rights (MDC) to review a complaint of discrimination based on disability. It is used to request an accommodation or modification to a policy, practice, or procedure in order to provide equal access for individuals with disabilities. There are three different types of Michigan Request to Review Accommodation: Request to Review Existing Accommodations, Request to Review New Accommodations, and Request to Review Reasonable Modifications. All requests must be completed and submitted to the MDC in order for the review to take place. Upon receiving the request, the MDC will review and investigate the complaint to determine if an accommodation or modification is appropriate. If approved, the MDC will provide the complainant with a written notice detailing the approved accommodation or modification.
