The Maryland Supplemental Page to DOM Rel Form (also known as the DR-312) is a document used in the state of Maryland in order to supplement a Domestic Relations Case Information Statement (DR-157). It is used to provide additional information regarding the parties involved in the case, including their addresses, assets, and liabilities. The form is available for download from the Maryland Courts website and must be signed by each party in the case. There are two types of Maryland Supplemental Page to DOM Rel Form: the DR-312A (for divorce cases) and the DR-312B (for custody cases). The form must be filed with the court, along with the Domestic Relations Case Information Statement (DR-157), in order for the court to consider the case.
