Maryland Address Change Request (Bilingual — Spanish Format) is a form used by Maryland residents to update their address information with the state. This form is available in both English and Spanish, and can be used to update the address for driver’s licenses, vehicle registrations, and voter registration, as well as to receive important mail from state agencies. There are two types of Maryland Address Change Request (Bilingual — Spanish Format): one for individuals and one for businesses. The individual request requires the name, previous address, new address, date of birth, and driver’s license or ID number. The business request requires the business name, previous address, new address, and contact phone number. Both forms must be signed and dated before being sent to the Maryland Motor Vehicle Administration or the Maryland State Board of Elections.
