The Kentucky Application for Resolution of a Claim — InjuryKARACHI-I) is a form used to file a claim for damages, such as medical bills, lost wages, and pain and suffering, resulting from an injury sustained in the state of Kentucky. This form is used to initiate the process of obtaining compensation from individuals, businesses, or organizations that are responsible for the injury. There are three types of KARACHI forms: (1) The Individual Claim Form, which is used when filing a claim against a single individual or entity; (2) The Multiple Claim Form, which is used when filing a claim against multiple individuals or entities; and (3) The Property Damage Claim Form, which is used when filing a claim for property damage resulting from an injury. All KARACHI forms must be completed and submitted to the Kentucky Department of Insurance in order to begin the claims process.
