The Kentucky Statement of Consent of Registered Agent is a document required by the Kentucky Secretary of State for a business entity to appoint a Registered Agent for Service of Process in the state. The Registered Agent is authorized to accept service of process on behalf of the business. The document must be executed by an officer or authorized member of the entity and filed with the Kentucky Secretary of State. There are two types of Kentucky Statement of Consent of Registered Agent. The first is for corporations, LCS, and other business entities, and the second is for foreign corporations. Both documents must include the name of the entity, the name and address of the Registered Agent, and the signature of an authorized member or officer of the entity.
