There are two types of Kentucky Notice of Cancellation of Reserved Name: (1) For-Profit Corporations and (2) Nonprofit Corporations. The For-Profit Corporation form includes the name of the entity, the name reservation number, the name reservation date, and the signature of the reserving agent. The Nonprofit Corporation form includes the name of the entity, the date of the name reservation, and the signature of the reserving agent. Both forms must be filed in order to cancel the reserved name.
