The Kentucky Affidavit and Beyond Control of School Evaluation Form are documents used by Kentucky school districts to record and report student absences for which the school is not responsible. These forms are used to document when a student has missed 10 or more consecutive days of school due to circumstances beyond the control of the school. The forms are used to inform the Kentucky Department of Education about the student's absence and must be completed and submitted to the district in order for the student's absences to be excused. Beyond Control of School Evaluation Form — This form is used to document when a student has missed 10 or more consecutive days of school due to circumstances beyond the control of the school.
