The Illinois Adult Sentencing Judgment / Probation Order (Short Form) is a form that is used in the state of Illinois to document the sentencing of an adult offender. The form outlines the sentence imposed, including the period of probation, the conditions of probation, and any fines and/or restitution imposed. The form includes the offender's name and address, the court's jurisdiction, the crime or offense they were found guilty of, the sentence imposed, and the date of sentencing. The form also includes an acknowledgment of the rights the offender has and the entry of a plea of guilty or no contest by the defendant. There are two types of Illinois Adult Sentencing Judgment / Probation Order (Short Form); one for felony offenses and one for misdemeanor offenses. The felony form includes greater detail, including the offender's criminal history, while the misdemeanor form is more concise. Both forms are available online or for download from the Illinois State Judiciary website.
