The Idaho Sexual Assault Examination Program Reimbursement Form is used to help reimburse medical and counseling costs for victims of sexual assault. The form is available through the Idaho Department of Health and Welfare and is designed to help victims of sexual assault access the medical and counseling services they may need in the wake of an assault. There are three types of forms available: the Idaho Sexual Assault Examination Program Reimbursement Form (for medical and counseling costs), the Idaho Sexual Assault Examination Program Reimbursement Form for Children, and the Idaho Sexual Assault Examination Program Reimbursement Form for Survivors of Human Trafficking. All forms require proof of identity and documentation of the assault in order to be processed.
