Physicians Report (Required By Florida Statutes, Section 744.3675) is a report from a licensed physician that is required when filing for a guardianship of a minor or incapacitated adult in the state of Florida. This report must include the physician's opinion on the mental and physical condition of the proposed ward, their capacity to manage their own affairs, and their current living situation.
