The District of Columbia General Forms (Adm)-In Form Paupers Service Waiver is a legal document used in the District of Columbia to waive service fees for pro SE litigants. It is a way for those who qualify as indigent to have their court fees waived. It is also known as an Affidavit of Poverty or Affidavit of Indigence. There are two types of District of Columbia General Forms (Adm)-In Form Paupers Service Waiver: a Petition to Waive Service Fees and a Waiver of Service Fees. The Petition to Waive Service Fees must be completed and signed by the indigent party or their attorney, and the Waiver of Service Fees must be signed by the court. Both forms must be submitted to the court in order to have the service fees waived.
