The Arizona Juvenile Court Cover Sheet is a form used to provide the Court with information about the case. It is filled out by court staff and used to identify the parties to the case, the charges, the assigned judge, and the attorneys involved. There are two types of Arizona Juvenile Court Cover Sheet: The Criminal Complaint Cover Sheet and the Juvenile Petition Cover Sheet. The Criminal Complaint Cover Sheet is used when a juvenile is charged with a crime and is filed with the court along with the criminal complaint. The Juvenile Petition Cover Sheet is used when a juvenile is accused of delinquency and is filed with the court along with the petition. Both forms include information about the juvenile, the nature of the offense, and the parties involved in the case.
