The Arizona Paternity Packet is a set of forms used by unmarried parents in Arizona to establish a legal relationship between father and child. The packets include forms to establish paternity, legitimize a child, and acknowledge paternity. There are three types of Arizona Paternity Packets: the Voluntary Paternity Acknowledgement Packet, the Legitimation Packet, and the Petition to Establish Paternity Packet. The Voluntary Paternity Acknowledgement Packet is used by unmarried parents to voluntarily establish paternity. The Legitimation Packet is used by unmarried parents to legally recognize a father's paternity rights and responsibilities. The Petition to Establish Paternity Packet is used by unmarried parents to establish paternity by filing a court petition. Each packet contains instructions, forms and a self-addressed stamped envelope to submit the forms to the Office of Vital Records.
