Arizona Chapter 7 Statement of Your Current Monthly Income is a form that debtors in Chapter 7 bankruptcy must complete to provide information about their current income. This form is used by the bankruptcy court to determine the debtor’s eligibility for a Chapter 7 discharge. The form must include the debtor’s gross monthly income from all sources, including wages, salaries, tips, bonuses, commissions, Social Security benefits, pensions, annuities, unemployment insurance, workers’ compensation, disability benefits, alimony, child support, and any other type of income. The form must also include the debtor’s average monthly expenses related to housing, food, transportation, and other needs. There are two types of Arizona Chapter 7 Statement of Your Current Monthly Income: Form 109C-1 for individual debtors, and Form 109C-2 for non-individual debtors.
