Alaska DV-125 (Multiple Petitioners) is a form used by the Alaska Department of Labor and Workforce Development to separate multiple petitioners into separate claims for Unemployment Insurance (UI) benefits. The form is used for those who are filing for UI benefits as a group, such as partners in a business, or a family which has experienced a sudden loss of income. There are three types of Alaska DV-125 (Multiple Petitioners). The first type is the Standard DV-125, which is used to separate multiple petitioners into separate claims for weekly UI benefits. The second type is the DV-125B, which is used to separate multiple petitioners into one or more joint claims for weekly UI benefits. The third type is the DV-125C, which is used to separate multiple petitioners into multiple separate claims for extended benefits.
