The New York Uniform Adoption — Attorney's Affidavit (Agency & Private Placement) is a document that is completed by an adoption attorney in order to provide a legal record of the adoption process. This document is used for both agency adoptions and private placements in the state of New York. The affidavit is signed by the attorney and includes the name of the child, the name of the adoptive parents, and the date of the adoption. It also includes a detailed description of the adoption process, including the home study, the consent of the birth parent or parents, and the receipt of a court order terminating parental rights. The affidavit must be filed with the state court in order for the adoption to be finalized.
