To enter Bermuda, your passport must be valid for the duration of your stay and have space for entry and exit stamps. Check with your travel provider that your passport and other travel documents meet requirements. Renew your passport if you need to.

They ask for your email and home address and where you're staying when on island. Dont worry, you won't be signed up for an email list and forever get spam in filling the form out.
