Renewing your passport by mail You can fill out Form DS-82 online, or. Download Form DS-82 PDF, 1.3MB and fill it out by hand, or. Find your local passport acceptance facility or regional agency and pick up a copy.
Form DS-82. You can print the form from the Department's public facing e-Forms webpage to ensure you are using the latest version. Search the form number in the search box at the top of the main page (DS-0082). The form is accessible online using Google Chrome (not Internet Explorer) to open the link.

You don't have to (it's now possible to renew your passport online), but you can. Most post offices offer passport renewal services—it's best to verify online if your local USPS is one of them. Note, however, that USPS are only passport acceptance facilities, they don't process passport renewal applications.
The first item appearing under Name/Nom is the surname (family name) of the individual followed immediately below by the first name (common name).
If you use a surname you must furnish the same here. No initials should be written and all initials (if any) in the applicant's name should be expanded. For instance, for the name used above, writing the Given Name as “P.K. JAIN” or “PIYUSH K JAIN” is not correct.
